If You Have Two or More Chronic Conditions, You May Be Eligible
Cosan partners with providers across the U.S. to help improve health outcomes of Medicare beneficiaries who have been diagnosed with two or more qualifying conditions.
Cosan Benefits
If you are eligible for Cosan, you may receive the following benefits:
- At least 20 minutes a month of care management services
- Monthly phone check-ins to help keep you on track and provide assistance
- Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more
- 24/7 access to your care plan via a Cosan patient app
- Expert assistance with setting and meeting your health goals
- Personalized assistance from a dedicated health care professional who will work with you to understand and stay compliant with your care plan
Does Medicare Pay the Costs?
These services are covered by Medicare up to 80% as well as by most other insurances, and could be covered in full by a secondary insurance (EXCLUDING Tricare and Humana insurance). Care Management is subiect to vour annual insurance deductible, coinsurance ($12-$15), and copay.
